Systems that worked when your organisation was small very often lead to problems later once the process has outgrown it’s origins. This sneaks up on companies and by the time the problem becomes obvious, they often don’t have the time or the expertise to review the process and get it working smoothly again. This can often be made more complicated by internal politics or by one department modifying it’s systems without due regard for how this affects other departments.
